Help! zoa seems to be dying

My zoa seems to be slowly dying, started with some black spots on the head than slowly spread and now some part turn white and seem to be dropping off. What is happening?

Nitrate: 5ppm

Phosphate: 0.05ppm

Calcium: 440

KH: 7dkh

Salinity: 1.023 (hydrometer)

Temp: 27.5-28.5 with fan

ATO to reduce salinity swings
